Qualifying Factors for Respite Stays at Ryan House
Ryan House provides respite, palliative and end-of-life care for those families with a child 17 years or
younger, diagnosed with a life-threatening condition and who could benefit from an overnight short-term
respite stay in Phoenix, AZ. Families are never charged a fee to utilize the services available. Our Care Team
works with each family to determine the qualification of each child. As we provide a variety of services, each
child is reviewed individually for their unique needs and qualification. Families may be asked to provide a
medical history and physical by their child’s primary care physician.

Respite Admission Orders
Families wishing to schedule a respite stay for their child may do so by completing the Respite Admission Order. The Respite Admission Order must be signed and dated by the child’s primary care physician within 24-hours of a respite stay. Download, print, and complete the Respite Admission Order with a signature from your child’s primary care physician.
Tour Our Home
Once you have spoken with a member of the Care Team and it is determined that the qualifying factors for a respite stay at Ryan House are met, we will invite you to take a tour of our home. A member of our Care Team will schedule a tour for you, your child, and your family to visit the house and learn first-hand about the programs, services and amenities our home has to offer.
