Families wishing to schedule a respite stay for their child may use this convenient link to complete the Respite Admission Order. While the Respite Admission Order must be signed and dated by the child’s primary care physician, some families prefer to complete as much of the information as possible prior to obtaining the physician’s signature (this will assist the physician’s office, too).
Unless there are changes to the child’s care routines or medication list, the Respite Admission Order may cover a period of six months.
